• Mass Covid-19 testing is being rolled out across all secondary schools in England.

• 1 in 3 people are believed to carry the virus but it is unknown as they are asymptomatic.

• The use of the lateral flow tests allow asymptomatic cases to be identified and therefore hopefully reduce the spread within our Academy.

• It will also mean that if your child is a close contact of a positive case, they can avoid isolating for 10 days by having a daily test.

• No child will be tested without consent. Consent can be withdrawn at anytime by contacting the Academy.

• You will receive a text message linking you to the appropriate form for your child. The message will be sent to the mobile number we have on file for parental/carer contact. Contact the Academy if you have recently changed your contact details.

BEFORE COMPLETING THE FORM:

► Carefully check you are completing the correct form for your child -double check the correct school and year group.

► You must fill separate forms for EACH CHILD you have at the Academy.
